Sylvester Stallone and Dolph Lundgren became two of the biggest action stars of the 1980s. Their paths first crossed on Rocky IV. Lundgren played the towering Soviet boxer Ivan Drago opposite Stallone’s Rocky Balboa.
Wanting the fight scenes to feel real, Stallone told Lundgren to throw actual punches during filming. One of them connected far harder than either expected.
“Dolph Lundgren put me in the hospital for nine days,” Stallone told The Hollywood Reporter. “I knew I was in trouble when I showed up and nuns met you at the ICU.”
The punch was an uppercut that caught his ribs. The force was so strong that Stallone’s ribcage slammed his heart. Doctors reportedly said that type of injury usually comes from head-on car crashes.
Despite the incident, Rocky IV became the highest-grossing film in the franchise. It also held the record for highest-grossing sports movie of all time for more than two decades.

Decades after Rocky IV, the two had another encounter during production of The Expendables in 2010. Stallone, who directed the film himself, harshly criticized Lundgren in front of the entire crew during one scene.
According to Variety, Stallone had Lundgren redo the scene roughly 20 times. International press was present that day, which added more tension. Lundgren later described the moment as deeply humiliating.
“We took a lunch break and I remember I was kind of in tears,” Lundgren told Variety.
“I called my wife that time and I basically just told her, ‘If he says one more word, I’m gonna knock him out and [expletive] this movie, I’m outta here. I’m just gonna punch him out and [expletive].”
Lundgren recalled how the pair managed to settle their dispute.
“And then I had a tap on the back and it was Sly,” he added. “And he was like, ‘Uh, I’m sorry about that. Let’s just do another take and let’s just keep moving on.’
“And we’ve had a few run-ins over the years, but what can I say, he’s a crazy Italian. I think he knows that I’ve always respected him and loved him, and I think that’s why we’re still friends.”
Lundgren has since appeared on Stallone’s reality series, and both reprised their Rocky roles together in Creed II. The two also reunited in three more Expendables sequels together.
