A Massachusetts jury is set to confront one of the most harrowing criminal cases in recent memory as the murder trial of Lindsay Clancy begins Monday, more than three years after prosecutors say she strangled her three young children inside the family’s home.

At the heart of the trial is a question that has divided legal and medical experts: Was Clancy a calculating killer, or was she gripped by a rare psychiatric illness that left her unable to understand what she was doing?

Prosecutors say the former Massachusetts General Hospital labor and delivery nurse carefully planned the Jan. 24, 2023, killings. They allege she sent her husband, Patrick Clancy, out on errands before strangling their children — Cora, 5, Dawson, 3, and 8-month-old Callan — inside their Duxbury home.

Her defense paints a starkly different picture.

Attorney Kevin Reddington argues Clancy was suffering from postpartum psychosis, a rare but severe mental illness that can develop after childbirth. According to the defense, she had spent months desperately seeking help as her mental health spiraled.

After the birth of her third child, Clancy reportedly became manic, struggled to sleep and was plagued by thoughts of harming herself. She sought treatment from specialists in postpartum mood disorders, was prescribed multiple psychiatric medications and eventually admitted herself to a psychiatric hospital. Nineteen days after being discharged, prosecutors say she killed her children.

Reddington has said Clancy told doctors she heard voices on the day of the killings telling her: “This is your last chance. Kill the children so you can kill yourself.”

After the children’s deaths, Clancy jumped from a second-story window, leaving her paralyzed from the waist down.

The closely watched trial is expected to last several weeks, with jurors hearing sharply conflicting testimony about Clancy’s mental state and criminal responsibility.

Boston criminal defense attorney and legal analyst Elyse Hershon said the verdict will likely hinge on whether jurors believe Clancy possessed the intent and mental capacity required for a murder conviction.

“Whenever there are deaths of children, it rattles emotions,” Hershon said. “Because these actions are incomprehensible and the outcome so horrific, people tend to want to hold someone responsible.”

Postpartum psychosis is rare, affecting an estimated one to two women per 1,000 births. Symptoms can include hallucinations, delusions, paranoia, severe mood swings and thoughts of harming oneself or others. Experts stress that the vast majority of women diagnosed with the condition never harm their children.

If convicted of murder, Clancy faces life in prison without the possibility of parole. If jurors find she lacked criminal responsibility because of mental illness, she would instead be committed to a state psychiatric hospital.

The case has drawn comparisons to other high-profile prosecutions involving postpartum psychosis, including Andrea Yates, whose murder conviction for drowning her five children in Texas was overturned before she was found not guilty by reason of insanity, and Dena Schlosser, who was also found not guilty by reason of insanity after killing her infant daughter.

Clancy and her former husband have also filed lawsuits accusing her medical providers of failing to properly diagnose and treat her deteriorating mental health before the killings.

The trial is expected to reignite debate over postpartum psychosis, with advocates arguing the condition is often misunderstood in the criminal justice system.

“Women who kill their children in the throes of this medical illness, postpartum psychosis, are seriously and profoundly mentally ill. They are not evil,” said Dr. Catherine Birndorf, founder of The Motherhood Center in New York. “To me it’s illness until proven otherwise. The fact that we consider it criminal until proven otherwise is problematic.”
